This peritoneal dialysis treatment program provides the opportunity to improve resident outcomes, develop staff skills and partner with other health professionals.

About this program

The organisation provides a continuum of care for 500 needy older Victorians.  Our resident had lived in independent living at our retirement village for many years.  For the past three years he has been on home dialysis.  Some months ago the resident found himself in the Austin Hospital and due to a range of medical issues facing the prospect of not being able to return to independent living and remaining in a hospital environment for the remainder of his life.

Our aim was to enable the resident to move into our residential aged care facility co-located within our retirement village setting.  This would enable him to continue to live in familiar surroundings and with his community of friends.

Working with the team at the Austin Hospital we were able to introduce a dialysis treatment program into an aged care environment, something as far as we are aware had not been previously undertaken in an aged care facility. Certainly the Aged Care Quality Agency assessment team were very complimentary of this innovative program during their support visit, noting they had never seen this service delivered in a residential aged care facility.
